Health Assessment

Hello, this task is from Neurologic System. Please fill in the entire patient history from pages 1 and from pages 2 to 4 in the examination, all the questions of the patient’s physical examination must be answered. You need a disease that has affected two cranial nerves that are described in the table and the rest of the cranial nerves that are normal. The teacher does not want them to only put normal but to describe that cranial nerve showing that it is fine. You do not have to fill out page 5. Please just complete pages 2 to 4
