K.B. is a 67-year-old man. He is 5’10” tall and weighs 260 lbs. He smokes a half pack of cigarettes each day and has been a smoker for the past 40 years. He has noticed that he is very tired during the day even though he sleeps 7-8 hours a night.

Vital signs temperature 98.7 orally, heart rate is 95 and regular, Respirations are 24 and unlabored. Blood Pressure is 152/84. Lungs are clear. Abdomen is large and rounded. Bowel sounds are positive in all four quadrants. No skin alterations. He is independent with all activities of daily living.

“I have been so tired during the day that I just can’t finish my work. Once I fell asleep driving home. I usually feel rested in the morning and never took naps in the daytime. I know I have put on a lot of weight. Could that be making me so tired?” Mrs. K. B. reports “He snores a lot at night. Sometimes he stops breathing, then he jumps and takes a deep breath. I’m the one who is tired.”

The physician orders a sleep study to be completed. The results showed sleep apnea. A CPAP machine is ordered to be used at night. The physician also encourages K.B. to stop smoking and to lose 50 pounds. K. B. agrees to try.
